In the case of idiopathic or functional testicular pain / when its cause is unknown or untreatable / conservative treatment should be attempted after a good 3 months. Conservative treatment includes physical rest, heat, appropriate or reduced sexual activity, wearing tight underwear and possible anti-inflammatory and analgesic medication / ibuprofen /.

If the pain persists despite these measures, a diagnostic blockade of the spermatic cord can be performed by injecting local anesthetics. This is an operation in the system of one-day surgery, the aim of which is to cut the spermatic branches of the n. genitofemoralis and n. ilioinguinalis. The success rate of microsurgical denervation in the treatment of chronic testicular pain is high and reaches up to 90% efficiency. Its effect is the permanent elimination of testicular pain. In extreme cases, orchiectomy can be performed for chronic testicular pain in older men.
