Procedure characteristics
What is microsurgical varicocele surgery actually?
Microsurgical varicocele surgery using the Marmar technique is an effective way to treat varicocele, i.e., varicose veins of the scrotum.
Varicocele treatment is necessary when the patient experiences pain or the appearance of the testicles changes and they become aesthetically disturbing.
The reason for varicocele treatment may also be male fertility problems. If a spermogram shows low or no sperm count, or poor quality, varicocele may be the cause.
What is varicocele and what are its symptoms?
Varicocele is a vascular disease that manifests as multiplication and dilation of blood vessels in the male scrotum. It often resembles varicose veins. Usually, varicocele is visibly manifested on the left side as swelling of the testicles. However, bilateral varicocele is not an exception.
Varicose dilated veins – varicoceles can be painful and cause unpleasant pulling sensations or pain in the scrotum in some patients. A common symptom of varicocele is reduced fertility to infertility in men. Infertility is caused by excessive heating of the testicle by dilated veins or impaired nutrition of testicular cells.
A subsequent complication is testicular hypotrophy, which causes gradual shrinking of the testicle and reduced testosterone production. Last but not least, varicocele can be an aesthetically disturbing factor that makes a man’s sex life unpleasant.
If it is bothersome or worsens male fertility, the best solution is microsurgical varicocele surgery.
How is varicocele diagnosed?
Varicocele diagnosis can be performed in several ways, all of which are painless. Initial diagnosis consists of palpation examination of the scrotum while standing. The doctor thus detects vein enlargement.
To confirm the diagnosis, you can undergo Doppler scrotal diagnostics, which consists of ultrasound examination, or the doctor will examine your spermogram and testosterone level.
Complications of microsurgical varicocele surgery
Microsurgical varicocele surgery carries minimal risk of complications. Rarely, chronic or intermittent testicular pain, so-called scrotal pain, may also occur. Testicular atrophy is equally rare.
What problems does microsurgical varicocele surgery solve?
The procedure solves varicocele and varicose veins of the scrotum.
What are the advantages of microsurgical varicocele surgery?
The advantage of microsurgical varicocele surgery is high effectiveness in relation to pain relief and fertility improvement, low recurrence of symptoms, less than 1.5% compared to classical surgery, where varicocele recurred in up to 30% of patients and laparoscopic surgery with 20% recurrence.
Another indisputable advantage is a significantly smaller wound and lower risk of secondary hydrocele, which is a common late complication after varicocele surgery.
What are the risks of microsurgical varicocele surgery?
Microsurgical varicocele surgery is considered the safest way to solve this problem, but like any surgical procedure, it carries certain risks. Most commonly, it involves minor swelling or pain at the incision site, possibility of bruising, and mild risk of wound infection.
In rare cases, varicocele may recur, but thanks to the Marmar technique, the risk of recurrence is minimal.
How does microsurgical varicocele surgery work?
The procedure is performed under general anesthesia. The surgeon makes a small incision in the groin area and under an operating microscope identifies and ligates the affected veins. Thanks to the magnification, it is possible to precisely preserve lymphatic vessels and the artery, which reduces the risk of complications. The entire procedure takes approximately 60 minutes and the patient is discharged home the same day.

Before microsurgical varicocele surgery, an internal pre-operative examination is required, which will be performed either by your general practitioner or by us.
Before the surgery, it is necessary to shave the scrotum, the area around the penis, and the groin. Microsurgical varicocele surgery usually requires subsequent 5-7 days of sick leave depending on the nature of the patient’s work.
The surgery is personally performed by MUDr. Milan Gajdoš, MPH, an experienced urologist with over 30+ years of practice, and MUDr. Michal Polák, an experienced pediatric urologist.

What happens after microsurgical varicocele surgery?
The surgery is performed under general anesthesia (artificial sleep) and usually takes up to 60 minutes. After the surgery, the patient goes home on the day of the surgery. The procedure does not require hospitalization. Since the surgery is performed under general anesthesia, we recommend that patients arrange for an escort.
After the surgery, the patient should observe 5-7 days of sick leave depending on the demandingness of the patient’s work.
Wound care in the groin is important. The patient should regularly re-dress the wound for 7 to 10 days. With each dressing, it is necessary to clean the wound with Betadine solution, dry with gauze, and apply Framykoin or Bactroban ointment, then cover with sterile gauze.
The patient will come for a check-up at our clinic in 10 to 14 days, as agreed with the doctor.
